I can confirm to the best of my knowledge that my Disability Assist Dog is fully vaccinated for my intended visit date
I will ensure that my Disability Assist Dog is wearing the proper and required harness/vest appropriate to the organisation it was certified by, upon arrival to the Park and for the duration of my visit
I will ensure that we adhere to the adapted Park map we are given on arrival
I can confirm that my Disability Assist Dog is in good health, i.e. no vomiting or diarrhoea in the past 24 hours
I understand that Orana Wildlife Park reserve the right to refuse entry if I don't meet the listed entry conditions or if my Disability Assist Dog misbehaves during my visit
